Requirements to be completed before the program start date.

  1. Applicants accepted to a program are responsible for completing a criminal background check. (Catalog pg.15)Catalog
  2. Applicants accepted to a program are required to complete and pass a substance abuse test.
  3. Applicants accepted to a program are required to ensure all required immunizations are complete.
    • Current seasonal flu shot
    • 2 MMR's (measles, mumps & rubella) vaccinations OR proof of immunities
    • History of chickenpox or proof of immunity
    • Documented proof of Tetanus Diphtheria Immunization
    • Documented proof of Hepatitis B series, or positive Hepatitis B titers. (you may be in the process of receiving your series) PLEASE NOTE:  As of May 2012 all students must have the Hepatitis B series completed before starting school. The Hepatatis B series may take 6 months to complete.
    • If you are not able to take a TB skin test due to a prior positive reaction or allergy, you will be required to have had a chest x-ray within the past 12 months.
  4. Applicants accepted to the program must provide official college transcripts (if applicable to program requirements) and a copy of their high school Diploma/GED.

                            **Transcripts must be mailed from school, we will not accept hand delivered transcripts**

  1. Applicants accepted to a program must provide verification of current health care insurance.  (Carelink is acceptable)
  2. Applicants accepted to a program must provide a current CPR for Health Care Providers certification by the American Heart Association.  (The American Red Cross is NOT acceptable)
  3. Applicants accepted to a program must pass a drug screen.
  4. Applicants accepted to a program must be able pass a background check
  5. All new students will be required to attend scheduled student success orientation prior to starting classes (does not apply to Perioperative, or Online and/or Seminar courses)
  6. International F1 Visa students must clear with the International Advisor prior to starting classes.
  7. All VA qualified students must clear with the VA/Financial Aid Advisor before starting classes.
